Premises Request: For use when reporting a premises fault or request for premises or caretaking attention:

HR Request: For use when creating a new role, terminating an appointment or varying a contract -  HR Request

When raising an HR request for appointment, please allow the following turnaround: 

 

  • A minimum of 3 days from raising the HR Request to the advert being placed (to allow approval) 
  • A minimum of 1 week from identifying the shortlisted candidates to HR Admin, to the interview date (to allow references to be sourced)
  • A minimum of 5 weeks from offer date to start date (to allow contracts to be processed)

ICT Hardware Order: For use when wishing to place an ICT hardware order form to position the Trust to support the requestor to identify the best value, group orders to identify best price or to ensure purchases are sensitive to the ICT strategy - ICT Request

Exit Interview: For use by an exiting staff member who would like to participate in the exit interview process but would rather complete a form than attend a face-to-face interview - Exit Interview

DPIA Process: For use when engaging a new supplier when there is a risk of compromising personal data of staff or children - DPIA Form
